Terms and conditions for Strides Membership 2x/week

FIT4MOM EWA BEACH MONTHLY MEMBERSHIP AGREEMENT
This contract is made between the client and FIT4MOM Ewa Beach, is a month-to-month agreement, and except as provided in the initial 10-day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited Stroller Strides/Stroller Barre classes.

AUTHORIZATION FOR PAYMENT
I authorize Hawaii Active Moms LLC to initiate a charge to the account entered into Front Desk/Pike13 for my monthly dues and non-refundable one-time registration fee. This authorization is to remain effective until I have cancelled my membership by emailing ewabeach@fit4mom.com.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM to fulfill my payment commitment and I am obligated to pay. Dues will be prorated from the date membership starts to the first of the following month. Dues from that point forward will be drawn monthly from the account on file on the first of every month. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

REGISTRATION AND NO SHOW POLICY
Clients must pre-register for the class they wish to attend. Classes will have a maximum enrollment of 10 and registration is on a first come first serve basis. Any client who registers but does not attend (NO SHOW) will still have the class deducted from their plan. Clients may un-enroll from a class up to 60 minutes before without penalty. Failure to do so will result in a single class deduction from their plan.

CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM administrator at ewabeach@fit4mom.com at least 15 days prior to their next payment due date to request a cancellation. Cancellations will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Ewa Beach franchise. Instructors are not authorized to cancel memberships.

HOLD POLICY
Memberships may be held for a minimum of 1 month and maximum of 3 months per hold period. The hold fee is $15 per hold period. To place a hold on this membership, the member must contact the FIT4MOM administrator at ewabeach@fit4mom.com at least 15 business days prior to their next payment due date to request a hold be placed on their membership, exceptions may be made for medical reasons. Holds will be confirmed in writing to the member once they have been processed. Holds can begin and end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume. It is up to the client to keep track of their return date. Instructors are not authorized to place a hold on memberships.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Ewa Beach within the initial ten days. You are responsible for paying for the classes already attended.

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.

COVID-19 PRECAUTIONS
Clients are required to pre-register for the live class they wish to attend (see REGISTRATION AND NO SHOW POLICY above). Clients are also required to fill out the COVID-19 Waiver and Release of Liability ahead of each class they plan to attend. The COVID-19 Waiver and Release can be found at covidscreen.paperform.co. Clients who do not fill out this form or have any signs or symptoms associated with COVID-19 may be asked to reschedule their class attendance without penalty.

Clients are expected to practice social distancing measures and remain 6 feet apart from anyone who is not in their household. Households or families may remain within 6 feet. Children under the age of 5 are required to remain in the stroller or carrier for the entirety of class. Children over the age of 5 must remain next to the stroller for the entirety of class. Failure to do this may result in a dismissal from class and will be treated as a NO SHOW and penalized as described above.

Please no sharing of snacks or toys during the entirety of the class.
